2021 Day of Action Agency Project Form

This year's Day of Action will be held Thursday, June 24, 2021 in partnership with the Highland Servant Leaders

Please fill out this form to be added to the 2021 Day of Action Project List. All forms must be submitted by Friday, April 23rd 2021 at 4:00pm.

Past projects have included: playground clean up and bike repair at King Community Campus,  clean up at Gramercy Park in East Dubuque, playground cleanup in Elizabeth, table repair at the Lena Community Center, food baskets delivered to senior citizens, organizing at the Galena Food Pantry and so much more! 


Please note, more than one project may be submitted, but projects will be selected based on the human and financial resources available to the United Way for Day of Action. 

Maximum of 3 projects. This does not guarantee all 3 projects. Projects are selected based on human and financial resources available.
